1147 - Development and Validation of a Mouth Handicap Scale in Systemic Sclerosis (MHISS)

Purpose: To develop a scale assessing the handicap induced by mouth involvement in systemic sclerosis (SSc) and to assess its reliability and construct validity.
Methods: A 34-item provisional scale was generated from patients mailed interviews (n=74), experts consensus (n=10), and literature analysis. Seventy two other SSc patients; 4 with limited SSc (lSSc), 38 with limited cutaneous SSc (lcSSc), 30 diffuse SSc (dSSc), fulfilling the American College of Rheumatology and/or Leroy & Medsger criteria were recruited. Test-retest reliability was assessed by use of the intraclass coefficient correlation (ICC). Divergent validity was investigated by use of the Spearman’s correlation coefficient. Factor analysis followed by varimax rotation was performed to assess the factorial structure of the scale.
Results: The item reduction process left 12 items with 5 levels of answers (range 0-48). The mean score of the scale was 20.3 ± 9.7. Test-retest reliability was 0.96. Hypothesis on divergent validity with global disability (HAQ, r= 0.33), hand function (Cochin hand function scale, r= 0.38), mouth opening (r= -0.22), depression (HAD, r= 0.18), and anxiety (HAD, r = 0.11) were confirmed. Factor analysis extracted 3 factors with eigenvalues of 4.26, 1.76, and 1.47 explaining 63% of the variance. These 3 factors could be clinically characterized. The first factor (5 items) represents the handicap induced by the reduction of mouth opening, the second factor (5 items) the handicap induced by the sicca syndrome, and the third factor (2 items) the aesthetic prejudice.Conclusion: We propose a new scale, the MHISS scale, with an excellent reliability and a good construct validity, that assesses specifically the handicap induced by mouth involvement in SSc patients.
