2021 Long Term Visual Outcome in Patients with Behçet’s Disease

Purpose: A chronic, relapsing bilateral uveitis involving both the anterior and posterior uveal tracts is a significant cause of morbidity in Behçet’s disease (BD) and it may cause blindness in up to 25% of patients. Our aim was to evaluate the course of ocular manifestations in patients with BD and to identify factors predictive of a poor visual outcome.
Methods: Twenty-nine BD patients, consecutively admitted at the Rheumatology Unit of the Second University of Naples were followed up through 10 years or longer (median 134 months). All the 29 patients, 18 males and 11 females, aging 27-66 years (mean 46.1±10.6) were classified according to the International Study Group for Behçet’s disease and underwent to ophthalmological evaluation every six months.
Results: Twenty-four (43 eyes) out of the 29 patients had ocular involvement (82.7%): in 3 anterior acute uveitis, in 10 posterior uveitis-retinal vasculitis, in 9 diffuse uveitis and in 2 anterior uveitis and retinal vasculitis occurred. Moreover, in 2 out of the 24 patients an optic nerve nerve atrophy developed. Among the 24 patients with ocular manifestations a monocyclic course of the ocular disease (less than 6 months duration) was registered in 9 patients, a polycyclic course (one or more relapses) in 4 and a chronic course (more than 6 months duration) in 11. Eight (12 eyes) out of the 24 patients became blind, all with retinal involvement: 1 with a polycyclic and 7 (11 eyes) with a chronic course (p=0.01). Moreover, a severe decrease in the visual acuity was registered in other 5 patients (6 eyes), 4 of them with a chronic course.
Conclusions: In BD patients, besides the retinal involvement, an ocular disease with chronic course may be considered predictive of a poor visual outcome.
