Session Overview:
"Medical Home" is a concept with six principles which have been applied to several chronic diseases both in adults and children. There are evidence based studies which demonstrate better outcomes and cost effectiveness of this model. We will discuss how it can be applied to patients with rheumatic diseases. In a time of fewer rheumatologists available to provide care, this model can allow physicians to provide effective interdisciplinary care and improve the lives of patients with rheumatic diseases.

Upon completion of this session, participants should be able to:

  • discuss the concept and principles of the medical home
  • apply these principles to patients with rheumatic diseases
  • cite evidence supporting the efficacy and cost effectiveness of this model
